Health plans and heath systems come together to provide patients with a quick and efficient way to assess symptoms and get a recommended care venue, appointment and cost comparison 24/7. Health plans struggle with how to provide members with real time information within the tools regulated for finding a venue of care. In particular the ability to assess symptoms and route members to the 'right' venue while providing choices based on cost and care recommendation from your mobile phone is the power of this concept.
What is shown here is an experience design concept I did as the lead for this project. This effort was subsequently branded by the research and design team for a demo for "XXX health care plan company". This project is currently under development for a large health care plan company. Company names remain anonymous due to propriety.

The plan member authenticates only at the very last possible gateway. This allows for crisis modalities of care. Once authenticated the data aggregates into their personal profile.
